FOR SERVICE TRIP APPLICATIONS:
Please print, sign, and mail the Service Trip Release Form along with the $200 non-refundable deposit to:

*Remember, the $200 non-refundable deposit is due four weeks prior to your program start date. The full, non-refundable program fee will be due two weeks prior to the start date.
FOR INTERNSHIP APPLICATIONS:

Upon acceptance to the program:
